    Bismillah hir-Rahman nir-Rahim !

    (Fatwa: 1497/1421=B=1430)

No one is exempted from forgetting and mistake. In the abovementioned case, if the imam stood up immediately after being prompted, the salah will be valid without any need for sajdah sahv. If he delayed for more than three tasbeeh (سبحان اللہ), the salah will be valid with sajdah sahv.  Neither it is needed to perform salah again nor to change the imam, but he can be admonished to lead salah mindfully.
