Question: If a woman satisfies herself till orgasm during her menses, is she required to perform ghusl twice? One for orgasm and other at the end of menses. Or just ghusl once at the end of menses is enough in this situation?

If the husband did intercourse with his wife during menstruation then only one ghusl shall suffice on the ending of menses. There is no need to do separate ghusl for intercourse and menstruation. However, according to Islam, it is unlawful and haram to have intercourse during menstruation, it must be avoided.

قال الله تعالى: وَيَسْأَلُونَكَ عَنِ الْمَحِيضِ قُلْ هُوَ أَذًى فَاعْتَزِلُواْ النِّسَاء فِي الْمَحِيضِ وَلاَ تَقْرَبُوهُنَّ حَتَّىَ يَطْهُرْنَ [البقرة : 222]

[Translation: They ask you about menstruation. Say: “It is an impurity. So, keep away from women during menstruation; and do not have intimacy with them until they are cleansed.]

عن أبي هريرة قال: قال رسول الله صلى الله عليه وسلم: من أتى حائضاً أو امرأة في دبرها أو كاهناًفقد كفر بما أنزل على محمد رواه الترمذي والدارمي وابن ماجة الخ (مشكاة المصابيح، كتاب الطهارة ، باب الحيض، الفصل الثاني، ص: 56، ط: المكتبة الأشرفية ديوبند)، قوله: "وجماعاً": أي: يحرمه وكذا ما في حكمه (رد المحتار، كتاب الطهارة، باب الحيض، 1: 485، مكتبة زكريا ديوبند).
